During your 1st visit you will need to bring some information with you including:

• Previous X-rays or MRI
  
  
• Copies of the films or reports
  If copies are not available the name of the center where they were taken
  
  
• List of medications you are currently taking
  
  
• If it is a workers compensation claim
  Case number
  Case managers name and phone number
  Any additional documentation provided by the employer or workers compensation insurance company
  
  
• If it is an auto accident
  Insurance company covering the injury
  Any additional documentation provided by the insurance company
